On 08ᵗʰ August, 2016, M/s. PIAC Flight PK 451 of (Airbus 320-214, Reg. No. AP-BLA) was a scheduled passenger flight from Islamabad to Skardu. After taxi out, the aircraft returned to ramp due to green hydraulic and brakes problem. The necessary rectification was done and the aircraft was declared serviceable for flight. The subsequent start up and taxi out remained uneventful, however, after takeoff, when the aircraft was climbing, the green hydraulic system fault appeared again. The aircrew decided to proceed back to Islamabad. After landing, the aircraft departed the runway after travelling approximately 8500 feet from beginni11g of runway 30 towards right side at 50 kts ingesting one of the PAPI lights by No. 2 engine.
