The flight crew mentioned at paras 2.1, 2.2. and 2.3. were operating PIAC Airbus A-300 AP-BAX PK-266 Karachi to Dhaka on a scheduled passenger flight on 16th July 1991. During start up at Karachi the pilot experienced a hot start on No. 1 engine. The EGT went upto 804 C. He reported the incident to maintenance on company's VHF frequency 131.6. Both Pilot and the maintenance decided that an inspection of the engine would be carried out by the engineers on board the aircraft at Dhaka. This was incorrect since the circumstances required an immediate shut down. The aircraft was however taxied out for runway 25 R which is 10,000' long. They had to hold behind an­ other PIA B-707 aircraft in front who was No. 1 for take off. During this period, the runway was being cleared of cowling and metal pieces that had fallen from No. 2 engine of another Airbus A-300 AP-BBA operating PK-300 Karachi to Islamabad.
