On 16th August, 1996 a bird bit incident occurred on M/s PIAC Airbus Reg No. AP-BBM while operating PK-465 fro:111 Lahore to Karachi. During layover checks the engineering staff after observing evidence of blood stains on the acoustic panel and outer ends of fan. blades of No. 1 engine decided to inspect it thoroughly. Subsequently, during detailed inspection by senior engineers evidence of piece of flesh a.bout ,one inch in size between 10 and 11 O'clock position at the fan outlet guide vanes· and few feathers at the .outer end·of outlet guide vanes were observed. Seeing the condition of the engine the senior engineering staff on duty was of the view that prior to release of subject engine for further operation its Boroscopic inspection should be carried out. Whereas, Director of Engine g and Maint felt no necessity of BSI under the circumstances-_and threatened the-senior engineers of dire consequences if a BSI on subject engine was carried out. Later on, same aircraft while opera g PK-312 at about 1300 hrs on the same c;la-yretµrned to_·bay from talce off point under instructions from CAA officials, Subsequently, BSI of No. 1 engine as required by CAA was carried out in which five first stage HP compressor blades were found damaged out of which one blade had evidence of material tear which is not permissible by the Maint Manual. Therefore, No 1 engine was removed.
